MEMORIAL WALL PLAQUE ORDER FORM

​Please note that each plaque is for an individual.  If you wish to order a plaque for more than one person, such as “your parents” then we have to omit the dates of birth and death since there won’t be enough space on the plaque. You always have the choice then of doing a separate plaque for each parent or grandparent.
