Guidance for Completing Member Information Form
You must enter the Member 1 email address.
If you are a new couple membership, please enter the information requested for both Member 1 and Member 2. If you are a new single membership, please enter the information requested for Member 1 only and leave the information for Member 2 blank.

Note: All data for a membership will be stored under Member 1’s email address, so please remember that email address for future reference.
Upon completion of the form, please click on the "Submit" button. After successful submission of the form, please click on the "Membership Payment" button below the form, and you will then be forwarded to a page to make or pledge your membership payment.
